I use this simple shortcut to dictate and log symptoms for a medical condition. It automatically creates time-stamped entries in a dedicated Apple Note called "Symptoms Log." If you don't already have a note by that name, the shortcut will set it up for you.
To make symptom tracking even easier, I've configured my iPhone's Action Button to trigger this shortcut instantly.
